If students/parents have the following paperwork, please bring them to the appointment:

• Birth Certificate OR government ID OR Form I-385 (issued at the border)

• Any and all report cards or school transcripts from previous education.  We accept pictures of official transcripts if you do not have the original.  

• Medical records and vaccination cards

• Assistance with enrollment

• Student IDs

• Assistance with filling out forms relating to students living in temporary housing (STLS)

• Screenings for students to determine eligibility for EL services and the current English proficiency levels

• Bus cards for students

• Information about support for students available through Chicago Public Schools

• Health Screenings for school

• Vaccinations offered on site (those necessary for school enrollment)

• Information about how to get help for other needs such as housing, medical, mental health support
